1. Pilentum Physiotherapy Privacy and Dignity Policy and Procedure

Each participant accesses supports that respect and protect their dignity and right to privacy.

1. Clients and their advocates are to be provided with this Pilentum Privacy Policy
   

2. The Privacy Policy is held online, available at time of booking, and in paper format to be given at first appointment or as requested.
   

3. Parent’s are to remain in the room when children under the age of 14 are being treated
   

4. All clinician’s are to hold current registration to Work with Vulnerable People.
   

5. Pilentum Physiotherapy adheres to the Code of Conduct for Registered Health Practitioners as follows

   1.  Information about patients or clients is to be treated as confidential.

   2.  Where practicable consent is sought, from patients or clients before disclosing information 

   3. Health records will be kept securely whether paper or electronic. When kept electronically, Cliniko is the method of secure, cloud based storage. Passwords, and two factor identification are in place.

   4. Sharing of health information with third parties such as insurers will occur with the clients consent, and as appropriate for their health care needs.

   5. Clients are provided with a curtained area to robe /disrobe with visual privacy. The door to the treatment room is to be closed during consults and interviews. The shared wall of the treatment room contains sound insulation.

   6. All staff are to maintain client confidentiality, and to respect client privacy.  Discussions about clients remain on a needs only basis for healthcare, and are to be respectful at all times.

   7. Clients are to be provided with the clinic privacy policy, and to consent to release of health information.

   8. We do not rent, sell or trade personal information to others.  

   9. If the clinic is sold your records will be transferred to the new owner, for use to provide allied health services.
      

I understand the privacy policy in place at Pilentum Physiotherapy and Wellness Clinic.  

I consent to the gathering and safe storage of my health information.

I consent to the use of cookies for online bookings, marketing and analytics.

I consent to the release of my health information between health care professionals where this is necessary for the benefit of my health care.  

I consent to the release of my health care information to Return to Work SA or Motor Vehicle Accident Commission and the CTP insurers as appropriate. 

I understand that personal information will be collected from third parties, such as 

• Private Health Insurers

• NDIS

• Third Party Government Agencies (like Medicare, DVA)

• Medical and health professionals (like General Practitioners, Allied Health Professionals, specialists etc)

• Solicitors, Compulsory Third Party Insurance and workers compensations companies
